Since 1973, Carroll County Health and Home Care Services, a private nonprofit organization, has been providing comprehensive home care services to individuals and families in Carroll County. Our goal is to promote the independence of the elderly and disabled in the comfort of their own homes.

Our agency provides and coordinates high quality home care services for individuals and families throughout Carroll County. Of New Hampshire's ten counties, Carroll County has experienced the greatest percentage of growth during the past decade. This has become a popular retirement area, and 17% of our 37,000 local residents are now 65 or older. About 25% of our clients have a primary diagnosis of dementia, and for many more it is a secondary diagnosis. To meet the needs of families affected by dementia, CCH&HCS has developed an Alzheimer's Disease Specialty Program. We provide education and training for our staff in the special needs of people with dementia, and also offer ongoing support and information for family caregivers. Throughout the past 25 years, we have responded to the changing needs of a growing elderly population in our area. Through internal planning and collaborative community networking, CCH&HCS has identified and tackled many of the evolving issues in home health care. Most importantly, we have added services and programs to help individuals maintain independence, preserve dignity, and promote real choice in keeping loved ones at home for as long as possible. Back To Top Services All of our services are arranged to meet the needs of the client and family and enhance their quality of life. Among those we offer are: Information & Referral In-Home Assessment Ongoing Case Coordination Personal Care/Health Aide Home Management/Homemakers Information & Referral: Since 1993, Carroll County Health & Home Care Services has offered a toll-free phone line to the residents of Carroll County in partnership with The Memorial Hospital. With our computerized database, Carroll County HealthLine offers callers the information they need in the area of health care, physician referrals and social services. The HealthLine website also offers a listing of monthly and special community health events, flu shot locations, and linkages to other health-related sites. In-home assessment: Upon receiving a request for home care services, we will arrange for a nurse case manager to make an initial home visit. This is a no cost/no obligation assessment visit. During this visit, our Case Manager assesses the needs of the client and family nembers, as well as home safety issues. The Case Manager will assist the family in choosing services that best meet their needs and will work with other agencies to assure continuity of services. At this time, issues such as staffing, scheduling and financial matters will be discussed. Ongoing case coordination: Once admitted to our service, clients are added to our weekly schedule of home care visits. Often, the visits are coordinated with other complementary services (such as physical or occupational therapy, or visiting nurses). Our Nurse Case Managers also arrange to meet with the client and family in order to assess needs and offer support on an ongoing basis. Should there be a need for additional services, the case manager will evaluate and follow up with the request. Our Home Care staff is trained to communicate with the Case Manager regarding any changes they might observe while providing services. Personal Care/Health Aides: Our Certified Nursing Assistants (CNA) are highly trained in meeting the personal care needs of our clients. Within our agency, there are three clinical levels that our CNAs can work toward. Our Level II CNAs are trained to assist our newer employees, to participate and provide input during staff meetings, and assume various leadership roles. Our personal care assistants have been trained, according to state guidelines, to assist clients with personal care as well as household needs. Home Management/Homemaker: Our Homemakers will assist clients with light housekeeping and home maintenance, although we do not offer heavy cleaning services. Each client's care plan indicates the cleaning tasks to be completed each week. Light vacuuming, dusting and mopping are common duties, as well as cleaning the bathroom and kitchen areas, bedding changes and laundry. Our Homemakers assist with shopping lists, weekly shopping and errands, as necessary. Our Homemakers can also assist with budgeting, mail, and correspondence. The Case Manager discusses homemaking options when the care plan is developed. Our Homemakers pay close attention to the care plan with regard to the client's dietary needs for shopping and meal preparation. Light meals are prepared for clients using the highest food safety standards. Homemakers also date food items and watch for refrigerator color spoilage. Back To Top Private Duty Home Care We are available to work with family members who need to provide extended coverage for a loved one. When incorporating large blocks of time into our schedule, we will often advertise, interview and train for "just the right" person to fill the hours and tasks. If the client or family has a friend, family member or neighbor who wants to provide the care, we can employ, train, and supervise the individual. Staff hired for individualized cases receive the same training, support and supervision as the rest of our employees. It is wise to anticipate a short delay between intake and the start of service when there is the need to hire additional staff for private duty care. (Please note: for your safety, CCH&HCS conducts reference, criminal background and motor vehicle checks on all new employees.) Back To Top Family Relief and Respite Care Many people find themselves as the primary caregiver for a loved one. Today, family caregivers often work outside of the home and need assurance that their loved ones are cared for during their work day. Others take care of a loved one full time and need an opportunity to get out for a few hours. We understand the challenges of caregiving and recognize the importance of helping family members with their caregiving duties. Respite time and services from CCH &HCS can help lighten your burden. Back To Top Caregiver Training and Support Our staff is trained to provide care, we also work with friends and family members who find themselves in the caregiving role. We help caregivers learn about safety issues and equipment, assisting with personal care needs, methods for relating to a loved one with dementia, education on the illness and, most importantly, is our ability to offer the much needed time off that a caregiver deserves. Family members also receive periodic mailings with supportive messages, information and ideas on caregiving. Back To Top Activities Program Our visits can be a wonderful opportunity for our clients to socialize. What could be better than playing a game, looking through photos, baking cookies or working together on a crossword puzzle. By assessing a client's interests our Nurse Case Managers can incorporate activities into the Care Plan. A grant from The Kendal and Anna Ham Foundation has provided us with games, puzzles, videos and craft supplies that our staff uses during visits. Newsletters with recipes, word puzzles, news items and holiday ideas are available to interested clients on a regular basis. Back To Top Nursing Home Placement Assistance While our goal is to help clients remain safe within their own homes for as long as possible, there are times when full-time nursing care is required. We can work with family members to find an appropriate placement in collaboration with area social workers, nursing homes and the Division of Elderly and Adult Services. Assistance is available with necessary paperwork, financial information, and doctor's orders.
