PANDEMIC PLANNING
Centers for Disease Control & Prevention - The CDC's Emergency Preparedness & Response web site. This site is intended to increase the nation's ability to prepare for and respond to public health emergencies.

NH Communicable Disease Control & Surveillance - The NH Department of Health & Human Service's Communicable Disease Control & Surveillance website. The Surveillance unit maintains the mandatory reportable disease system and is responsible for collecting, analyzing, interpreting and reporting NH infectious disease data. The Disease Control unit is responsible for infectious disease control activities, case follow-up, patient and provider education and disease outbreak investigation.

US Government Pandemic Planning - One-stop access to U.S. Government avian and pandemic flu information. Managed by the Department of Health and Human Services.

World Health Organization - The World Health Organization's Epidemic and Pandemic Alert and Response website.

AsthmaNow: NH Asthma Control Program - A website that brings together information and resources to educate and support people in New Hampshire with an interest in asthma. A collaborative effort of the NH Asthma Control Program, American Lung Association of New Hampshire, Chronic Conditions Information Network of Vermont and New Hampshire (CCIN), and Southern NH Area Health Education Center (AHEC).

American Heart Association - The official website of the American Heart Association provides a wide range of information on heart disease, heart health, self-help and statistics.

Centers for Disease Control & Prevention - The Centers for Disease Control and Prevention (CDC) is recognized as the lead federal agency for protecting the health and safety of people, at home and abroad, providing credible information to enhance health decisions, and promoting health through strong partnerships. The site includes information on disease prevention and control, environmental health, and health promotion and education activities.

Centers for Medicare & Medicaid Services - Official website of the Centers for Medicare & Medicaid Services (CMS), the federal agency within the U.S. Department of Health and Human Services that runs Medicare, Medicaid, and the State Children's Health Insurance Program (SCHIP).

Mayo Clinic Health Info - An excellent source of accurate, reliable and easy-to-understand health information on the Web. Find information fast on diseases and conditions or browse featured articles, tools, recipes and timely topics.

MEDLINEplus.gov - Health information from the National Library of Medicine at the National Institutes of Health. Accurate, current, commercial free and totally private health information. Health topics, encyclopedia, drug information, clinical trials, and the latest health news.

National Mental Health Information Center - The US Dept. of Health & Human Services website with consumer information on mental health and substance abuse programs, services and support.

ServiceLink of New Hampshire - A statewide network of 13 community-based sites providing connections to resources for seniors, adults with disabilities and chronic illness, their families and caregivers. Call the local Carroll County ServiceLink office (toll-free) at 1-866-634-9412 or 603-323-9394, or visit the state ServiceLink website. ServiceLink is sponsored by the NH Department of Health & Human Services in cooperation with local agencies.

CANCER SUPPORT
American Lung Association - The official website of the American Lung Association provides a wide range of information on lung disease, asthma, tobacco use, air pollution, self-help and statistics.

Breast & Cervical Cancer Program - "Let No Woman Be Overlooked" - A free screening for breast and cervical cancer for women 18-64 through the New Hampshire Dept. of Health & Human Services Office of Community and Public Health. Call 1-800-852-3345, Ext. 4931 for eligibility and participating sites.

Chemotherapy and Beyond - A nonprofit program of the Scott Hamilton CARES Initiative, authored by the Cleveland Clinic. Scott Hamilton's chemotherapy site offers chemo information for cancer patients and their families, caregivers and friends.

Jen's Friends Cancer Foundation - A local cancer foundation that provides support to those with cancer, especially with the financial hurdles associated with cancer treatment. PO Box 1842, North Conway, NH 03860, (603) 356-5083.

National Cancer Institute - The National Cancer Institute is the lead federal agency for research on the causes, prevention, early detection, diagnosis and treatment of cancer. Their website contains information about the diagnosis, prevention, and treatment of cancer, rehabilitation from cancer, and the continuing care of cancer patients and the families of cancer patients.


HEALTH DATA RESOURCES
NH Health Data Inventory - New Hampshire Health Data Inventory has been created to provide an inventory of existing health data to New Hampshire communities. The purpose of this inventory is to connect community residents to health data sources and to the owners of these data sources. If community residents need assistance using these data sources they are encouraged to contact the data owners who will be willing to assist them.
