Welcome to Visiting Nurse & Hospice Care Services of Northern Carroll County. Our agency offers a comprehensive range of services that brings high quality health care and supportive services into the homes of our clients 24 hours a day. Our services help bridge the care continuum from the hospital to the home, allowing patients to recuperate safely in their own comfortable surroundings.

Home care has always meant "high touch" care. Today, it means "high tech" care, too. Whether a person has a simple or complex condition needing short-term or long-term care, we can provide quality care and assistance in the home.

Historically, "hospice" meant a place of shelter and nourishment for travelers on a difficult journey. Today, hospice stands for a concept of caring which provides physical, emotional and spiritual care to people with a life-threatening illness and their families. Having hospice as part of our agency services helps us better serve our clients throughout their lives, with respect, compassion and dedication to quality care.

  • Occupational therapy
  • Speech therapy
  • Medical social service
  • Hospice care and volunteer support services

  • Blood pressure screenings
  • Bereavement support group
  • Health education
  • Public flu and pneumonia shot clinics
  • "Smart Start" – Maternity home visits


Home Care
We offer home health care to all residents in the towns of northern Carroll County. A family or client may request our services to make an initial assessment visit. Subsequent visits are made with the order of your family physician. Your physician or hospital may also make a referral directly to us on your behalf.

Hospice Care
Hospice care involves a core interdisciplinary team of professionals and volunteers who provide medical, psychological and spiritual support for people with life-threatening illness and their families. Hospice has a special focus on pain and symptom management so that families are able to remain together in the comfort and privacy of their own homes.

Hospice treats the person instead of the disease; its philosophy emphasizes quality of life and involvement of the family. It brings together the knowledge and skills of professionals such as physicians, nurses, home health aides, social workers, spiritual caregivers, counselors and trained volunteers.

Hospice volunteers are specially trained to:
  • Be someone who can listen compassionately and supportively
  • Offer respite care to relieve family members at set intervals
  • Supplement the services of medical professionals, clergy and public agencies
  • Help and counsel grieving family members after a loved one’s death

Hospice services are covered by Medicare and most private insurance and managed care plans.

"Smart Start" - New Baby Home Visit
Smart Start is a home visitation program sponsored jointly by VNS and The Memorial Hospital’s Family Birthing Center. Every new mother is eligible for the home visit program, which will be discussed during your hospital. New mothers and their babies receive a visit in the comfort of their home within 1-2 weeks after leaving the hospital. Your home visit will include a complete professional nursing assessment of you and your baby. Feel free to discuss any questions you may have on newborn care, parenting issues, mother’s health needs, referrals for follow-up care, and resources available in the community.

If you live in Bartlett, Jackson, Glen, Conway, Chatham, Albany, Eaton, Madison, Tamworth or Freedom, you will receive your visit from the VNS of Northern Carroll County. Mothers from other towns will be served by the VNS of Southern Carroll County, Berlin’s Androscoggin Valley Home Health Agency, or Bridgton’s Community Health Services, all of who provide the same service.

Our Staff
Our professional staff is trained to care for people of all ages, and to coordinate care with other community resources. Registered nurses manage all of a patient’s health care needs, coordinate care with physicians, and recommend additional resources if necessary.

They perform health and home safety assessments, administer medical treatments and procedures, and teach patients and their families how to care for their health needs.

Comprehensive social work, rehabilitation and personal care services are also available and coordinated by our staff. When treatment is no longer an option, our hospice staff can offer families support, dignity and comfort. Trained and caring hospice volunteers support and assist our staff in meeting the special needs and requirements of each hospice client and family.

Fees
We are a private non-profit organization licensed by the State of New Hampshire and supported by our local communities. Charges for services are based on the actual costs of making staff available to communities. When a client’s required treatment is covered by Medicare, Medicaid or private insurance, the fee is paid by that party. If insurance coverage is not available, then fees are adjusted according to the client’s ability to pay. For more information about our fees, please call our office.

Certifications and Memberships
Visiting Nurse & Hospice Care Services of Northern Carroll County is licensed by the State of New Hampshire, Medicare certified, and a member of the National Home Care Association, Home Care Association of New Hampshire, and the National Hospice Organization.

We are also a member of the Memorial Development Foundation whose affiliates include The Memorial Hospital, Carroll County Health & Home Care Services, Merriman House, and White Mountain Community Health Center.
